If you are submitting a 5-digit zip code on your claim your carriers will return the claims as unprocessable every time. If an insurer returns your claim as unprocessable, check your denial reason against the following Remittance Advice and Remark codes:
- Reason Code 16 – Claim/service lacks information that is needed for adjudication. Additional information is supplied using remittance advice remark codes whenever appropriate.
- Remark Code MA130 – Your claim contains incomplete and/or invalid information, and no appeal rights are afforded because the claim is unprocessable. Please submit a new claim with the complete/correct information.
- Remark Code MA114 – Missing/incomplete information on where the services were furnished.
